Admission:
  • Get and fill-out the enrollment procedure & registration forms
Program Chairperson/Dean:
  • Present grades for evaluation
  • Approval of courses
Registrar:
  • Present approved enrollment form
Hotel Lobby:
  • Purchasing cloth for uniform
Finance Office:
  • Windows 1 & 2 for payment
Registrar Office:
  • Printing of Certificate of Registration
Office of Student Affairs:
  • ID Picture Taking and Printing

Enrollment Procedure for New Students

Admission:
  • Checking of requirements
  • For incoming first year students:
    • Report Card
    • Certificate of Good Moral Character
    • Birth Certificate-PSA/NSO(photocopy)
  • For transferees:
    • Transcript of Records(photocopy)
    • Honorable Dismissal/Transfer Credentials
    • Certificate of Good Moral Character
    • Birth Certificate-PSA/NSO(photocopy)
